At Synthesis, safety is our top priority. For this reason we require everyone whose last Health Screening took place over six (6) months ago to complete a Health Re-screening Form to participate in the Immersion program.

The Health Re-screening form asks you to report any changes in your mental and physical health since your last submission of the Health Screening Questionnaire. We estimate that it will take between 15 and 20-minutes to complete and it is imperative that you answer all questions truthfully. We kindly ask you to be as detailed as possible in your responses. Participation in our programs and trainings will be will be discontinued/denied if information is withheld or falsified.

Your Health Screening data will be handled to the highest standards of confidentiality. We store your answers in compliance with GDPR and HIPAA, and will only use your data for assessment purposes. Data Safety at Synthesis Explained
